**Job Summary:**

We are seeking a Patient Care Support Specialist to join our team at Rhynocare Staffing Solutions. This is an exciting opportunity for individuals who are passionate about delivering high-quality patient care in a dynamic and supportive environment.

**Key Responsibilities:**

  • Observe patients assigned to your care, ensuring their safety and well-being
  • Provide 1:1 compassionate supervision services for residents with dementia, Alzheimer's, frontal brain injury, schizophrenia, stroke, etc.
  • Assist patients with activities of daily living under the close or direct supervision of registered health professionals
  • Document observations and interactions using standardized tools, such as the Dementia Observations Scale (DOS) and Behaviour Assessment Record (BAR)
  • Notify healthcare teams immediately if you identify any risks or changes in patient behavior or condition
  • Maintain a professional image and wear a name tag easily legible for residents and visitors

**Requirements:**

  • Grade 11 or equivalent education as recognized by the Ministry of Education, Province of Ontario
  • One year of recent related experience in patient care, hospital, long-term care, or complex continuing care settings required OR a Personal Support Worker certificate from an accredited program OR completion of a minimum of one year of an accredited Nursing program
  • Fluency in speaking and documenting in English
  • Current Non-Violent Crisis Intervention Certification
  • Proof of current immunization for measles, mumps, rubella, varicella (MMRV), diphtheria, pertussis, and hepatitis B
  • Proof of negative 2-step TB skin test within the past year or negative chest x-ray within the past 5 years

**Estimated Salary:** $45,000 - $55,000 per annum, depending on experience and qualifications
