Experience with care of patients with cardiac disease, specifically in the care of patients with heart failure.Experience with telemonitoring will be valuedExperience with patients and family self-care education will be valued.

Incorporate telemedicine in the follow-up of patients with HF.-
Establish a structured discharge follow-up with special emphasis on the vulnerable post-discharge phaseProvide health education to patients
