**JOB DESCRIPTION**

**Education and qualifications**:
**Required: **

Nurse 10 hours’ weekly

**Experience and knowledge**:
**Required: **

4-5 years of experience with care of patients with cardiovascular research disease, specifically in the care of

patients with ischaemic heart disease and heart failure.

2 years of experience with telemonitoring will be valued

4-5 years of experience with patients and family self-care education will be valued.

**Main responsibilities and duties: **

Incorporate telemedicine in the follow-up of patients with cardiovasculars disease.

Establish a structured discharge follow-up with special emphasis on the vulnerable post-discharge phase

Provide health education to patients
