Job Summary:
We are seeking an experienced Ambulatory Care Registered Nurse to join our team at Trinity Health Mid-Atlantic. As a Care Manager, you will play a critical role in supporting our offices in Cohoes and Waterford, focusing on the health and well-being of our Trinity Health medical group patients with multiple chronic diseases.
Key Responsibilities:
- Focusing on disease education and identifying and mitigating social influencers of health.
- Coordinating needed services and collaborating with provider offices to ensure integrated care through the healthcare continuum.
- Developing and implementing care plans that improve health outcomes for our patients.
Requirements:
- BSN required (NYS Registered Professional Nurse licensure).
- Previous experience required in care management in at least one of the following settings: disease management, home health care, hospital, or physician office setting.
- Preferred Certification in Case Management (CCM).
- 5 years of RN experience in Care Management required.
- Prior electronic medical record (EMR) experience required.
- Previous project leadership experience required.
Preferred Qualifications:
- Strong community resource knowledge.
- General knowledge of insurance plans and clinical resource management.
- Excellent oral and written communication skills.
- Excellent customer service skills.
- Ability to work independently, setting priorities to coordinate care plans efficiently.
- Proven leadership skills and ability to work effectively in a team environment.
- Highly organized and detail-oriented, with the ability to perform multiple tasks.
What We Offer:
- A competitive salary range of $44.95 per hour, based on experience, skills, and education.
- Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
