About the Job

We are seeking a skilled and compassionate Hospice LPN to join our team. As a Hospice LPN, you will be responsible for providing bedside care to patients, supporting families and caregivers, and working closely with the RN Case Manager. You will be an integral part of our interdisciplinary team, working together to provide comprehensive care to our patients.

Responsibilities

  • Provide bedside care to patients, including administering medications, wound care, and other treatments as needed
  • Support families and caregivers, providing education and guidance on patient care and management
  • Work closely with the RN Case Manager to develop and implement patient care plans
  • Participate in interdisciplinary team meetings to discuss patient care and progress
  • Document patient care and progress in accordance with regulatory requirements

Qualifications

  • Graduate of an approved nursing program
  • Valid Ohio Licensed Practical Nurse (LPN) license
  • CPRE certification
  • A minimum of 2-4 years' direct patient care experience in hospice or home care
  • Licensed driver with a valid automobile insurance policy
