Job Summary

At Otterbein Seniorlife, we are seeking a skilled and compassionate Nurse Case Manager to join our team. As a key member of our Hospice Care team, you will provide high-quality, patient-centered care to individuals in need of end-of-life support.

Responsibilities

  • Provide skilled professional nursing care to patients, their families, and caregivers, as prescribed by the physician and in compliance with regulatory requirements.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
  • Provide education and support to patients, families, and caregivers on disease management, symptom control, and end-of-life care.

Qualifications

  • Bachelor's degree in Nursing preferred.
  • Valid Ohio Registered Nurse (RN) license.
  • A minimum of 2-4 years' direct patient care experience in hospice or home care.
