Job Summary

We are seeking a passionate and dedicated Direct Support Professional to join our frontline team. As a Direct Support Professional, you will play a vital role in supporting individuals with IDD to lead self-determined and self-directed lives, fully participating in their communities.

Key Responsibilities
  • Assist individuals with IDD in identifying their wants and needs, and develop and maintain skills to facilitate their full potential.
  • Support individuals in accordance with their Person-Centered Plan (PCP) or Individual Supports Plan (ISP), and Makom's Contract for Supports.
  • Provide individualized supports and coaching with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) as required.
  • Ensure safe and clean living and/or working environments, and report any maintenance needs or assist individuals to obtain assistance from their property management as needed.
  • Be familiar with and comply with medical assessments, dietary restrictions/allergies, care plans, and recommendations provided by medical professionals.
Qualifications
  • A minimum of a high school degree, with further education or an associate degree in a human services-related field being helpful.
  • Ability to complete required training within the allowed time period.
  • Ability to lift up to 100 pounds with assistance, and be capable of kneeling on the floor to help individuals they support.
  • Ability to solve problems, think critically, and provide excellent customer service.
  • Effective communication skills with individuals with IDD, Makom leadership, peers, families, advocates, and external constituencies.
  • Proficiency with Microsoft applications and computer skills.
  • Ability to work independently and with others, exercising professional judgment.
