- Identify and mitigate social influencers of health, coordinating needed services and collaborating with provider offices to ensure integrated care through the healthcare continuum.
- Work closely with healthcare teams to ensure seamless transitions of care and improved health outcomes.
- Develop and maintain strong community resource knowledge to provide patients with access to necessary services and support.
- Collaborate with patients, families, and healthcare teams to develop and implement personalized care plans.
- Provide education and support to patients and families on disease management, self-care, and health promotion.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- BSN required (NYS Registered Professional Nurse licensure).
- AD will be grandfathered and must meet performance standards.
- Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting.
- Preferred Certification in Case Management (CCM).
- 5 years of RN experience in Care Management required.
- Prior electronic medical record (EMR) experience required.
- Previous project leadership experience required.
- Excellent oral and written communication skills.
- Excellent customer service skills.
- Ability to work independently, setting priorities to coordinate care plan efficiently.
- Proven leadership skills and ability to work effectively in a team environment.
- Highly organized and detail-oriented, ability to perform multiple tasks.
- Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support.
- Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences.
- Incorporate strategies for interacting with persons from diverse backgrounds (e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, and affiliation, mental and physical capabilities).
- Competitive salary range: $44.95 per hour.
- Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
- The pay range may also vary within the stated range based on location.
