Position Overview:
We are seeking a highly motivated and compassionate Registered Nurse (RN) Care Coordinator to join our team in the San Fernando Valley and neighboring areas. As an RN Care Coordinator, you will be responsible for assessing seniors and determining the level of care required for our homecare services. This is an in-field position that involves assessing patients/participants' needs and taking vital signs. Bilingual Spanish skills are a plus for effective communication with our diverse patient population.

**Responsibilities**:
1. Conduct comprehensive assessments: Perform thorough assessments of seniors to determine their healthcare needs, including physical, mental, and emotional aspects.

2. Evaluate level of care: Assess and determine the appropriate level of care necessary for each patient/participant, considering factors such as medical history, current medical conditions, and functional abilities.

3. Take vital signs: Accurately measure and record vital signs, including blood pressure, heart rate, temperature, and respiratory rate.

4. Develop care plans: Collaborate with the healthcare team to develop personalized care plans that address the specific needs of each patient/participant.

5. Coordinate services: Coordinate and facilitate the delivery of necessary homecare services, including arranging for skilled nursing visits, therapy sessions, medical equipment, and other required services.

6. Monitor patient progress: Regularly evaluate and monitor the progress of patients/participants, ensuring that care plans are effective and adjustments are made as needed.

7. Provide patient education: Educate patients/participants and their families regarding their health conditions, medications, and self-care techniques to promote independence and wellness.

8. Collaborate with healthcare professionals: Maintain effective communication and collaboration with physicians, therapists, social workers, and other healthcare professionals involved in the patient's care.

9. Maintain documentation: Accurately document assessments, care plans, interventions, and other pertinent information in electronic medical records, ensuring compliance with privacy regulations.

10. Stay updated: Stay informed about current healthcare practices, regulations, and guidelines to provide the highest quality of care and support to patients/participants.

**Qualifications**:
1. Registered Nurse (RN) license: Possess a valid and active RN license in the state of California.

2. Minimum of two years of field experience: Have at least two years of experience working in a healthcare setting, specifically assessing patients/participants' needs and taking vital signs.

3. Bilingual Spanish (preferred): Proficiency in Spanish is highly desirable to effectively communicate with Spanish-speaking patients and their families.

4. Strong assessment skills: Demonstrated ability to conduct thorough assessments and accurately evaluate patients/participants' needs.

5. Excellent communication skills: Effective verbal and written communication skills to interact with patients/participants, families, and the healthcare team.

6. Compassion and empathy: Display a genuine concern for the well-being of seniors and their unique healthcare needs.

7. Organizational and time management skills: Ability to prioritize tasks, manage multiple responsibilities, and meet deadlines in a fast-paced environment.

8. Attention to detail: Meticulous attention to detail to ensure accurate documentation and implementation of care plans.

9. Team player: Collaborative and cooperative approach to work, with the ability to work effectively within an interdisciplinary team.
