We are seeking an experienced Care Coordinator Specialist to join our team. As a key member of our management team, you will play a vital role in ensuring the delivery of high-quality care services to our customers.
**Key Responsibilities**
As a Care Coordinator Specialist, your primary responsibilities will include:
- Conducting comprehensive assessments of customer needs and creating personalized care plans that meet their goals and expectations
- Collaborating with cross-functional teams to ensure seamless delivery of care services
- Providing exceptional customer service, responding promptly to customer inquiries and concerns
- Maintaining accurate records and reports, adhering to regulatory requirements and company policies
**Requirements**
To be successful as a Care Coordinator Specialist, you must have:
- Proven experience in care coordination or a related field
- Excellent communication and interpersonal skills, with the ability to build strong relationships with customers, staff, and stakeholders
- A strong understanding of care principles, regulations, and industry standards
- The ability to work autonomously, with minimal supervision, and prioritize multiple tasks effectively
- Relevant qualifications, such as NVQ Level 3 in Health and Social Care or equivalent
**What We Offer**
