Physical Therapist Outpatient Neuro PRN

We are seeking a skilled Physical Therapist to join our team in an Outpatient Neuro PRN position. As a Physical Therapist, you will be responsible for evaluating and administering treatment procedures to patients referred for physical therapy. You will also direct and guide patient/family teaching, provide oversight for ancillary personnel, and interact with other members of the rehabilitation team to coordinate goals and improve quality of care.
Responsibilities- Evaluate patients and document evaluation in EMR timely.
- Re-assess and revise goals and treatment plan as indicated by patient's status, program requirements, and CMS rules and regulations.
- Evaluate patient progress toward expected outcomes and document as indicated regarding subsequent assessments, evaluation of treatment and interventions including responses.
- Plan and implement age-appropriate patient care and activities consistent with the medical plan of care of each patient, including discharge planning.
- Collaborate with physician regarding service needs.
- Keep daily statistics and charges as established for each area.
- Contribute to department Quality Management activities providing recommendations for systematic and programmatic improvements based upon the results of the data.
- Document results of standardized assessments and act as a resource to Program Managers and Directors for achievement of standards for accreditation by CARF, Commission on Cancer, and/or other accrediting organizations.
- Provide guidance to new and experienced personnel, and perform lead therapist duties, when assigned.
- Communicate effectively in order to provide optimal care to the patient and to promote a professional climate throughout the hospital.
- 1 year of experience as a Physical Therapist.
- Licensed as a Physical Therapist by the Louisiana Physical Therapy Board.
- American Heart Association, Basic Life Support Health Care Provider.
