Job SummaryWe're seeking a skilled Registered Nurse to provide patient care to an assigned group of patients in our Home Health department. As a key member of our team, you'll work under the direction of an RN to deliver high-quality care to patients with cardiac, medical, surgical, and post-critical care needs.
Responsibilities- Provide patient care to an assigned group of patients according to the multidisciplinary plan of care.
- Work with patients under the direction of an RN to deliver care that meets their unique needs.
- Facilitate and maintain a safe environment for patients and others.
- Perform priority technical skills and procedures required by the area/department/assignment safely, effectively, and efficiently.
- Utilize the nursing process to deliver patient care, establishing and/or revising priority for patient care activities based on acuity of needs, resource availability, patient preference, and other departmental needs.
- Contribute to the development of a plan of care for assigned patients and collaborate with interdisciplinary team members to identify actions to improve patient care.
- Initiate and/or maintain interventions to assure continuity of safe and effective care of patients, including medication management and patient education.
To be successful in this role, you'll need to demonstrate problem recognition and management in response to data indicating potential risk to individual's health, initiate action to correct, reduce, or prevent the risk, and give a rationale for actions or decisions based on scientific principles, successful experience, intuition, and/or standards of practice.
You'll also need to maintain documentation consistent with established guidelines and comply with established hospital, departmental, and personnel policies.
