Social Worker

Som Fundació De Suport A Persones Amb Discapacitat is a non-profit organization dedicated to supporting individuals with intellectual disabilities. We are seeking a highly motivated and experienced Social Worker to join our team in Tarragona and Penedès.

We are looking for a skilled and compassionate Social Worker to provide person-centered support to individuals with intellectual disabilities. The successful candidate will work closely with our team to develop and implement personalized plans, ensuring the best possible outcomes for our clients.

• Conduct regular assessments and develop personalized plans to support individuals with intellectual disabilities.
• Provide guidance and support to individuals and their families to promote independence and inclusion.
• Collaborate with healthcare professionals, educators, and other stakeholders to ensure comprehensive support.
• Develop and maintain relationships with community resources and services to enhance support options.
• Monitor and evaluate the effectiveness of support plans, making adjustments as needed.
• Participate in team meetings and contribute to the development of new initiatives and programs.

• Diploma in Social Work or related field.
• Minimum 3 years of experience working with individuals with intellectual disabilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office and other relevant software.
• Valid driver's license and access to a vehicle.
