Disease Specific Certification Coordinator

We are seeking a highly skilled and motivated Disease Specific Certification Coordinator to join our team at LCMC Health. As a key member of our quality improvement team, you will play a critical role in ensuring the highest standards of patient care and safety.
Key Responsibilities- Coordinate and implement performance improvement initiatives to enhance patient outcomes and reduce preventable harm.
- Develop and maintain performance improvement project charters, timelines, and key deliverables.
- Collaborate with interdisciplinary teams to identify opportunities for quality improvement and implement evidence-based practices.
- Facilitate multidisciplinary teams to reduce hospital-acquired conditions and improve patient safety.
- Perform data abstraction and analysis to inform quality improvement initiatives.
- Prepare summaries and presentations for physician reviewers and enter data into computer systems for statistical compilation.
- Assist in developing and revising clinical policies and serve as a resource for ongoing development of job descriptions and competencies.
- Coordinate compliance with regulatory agencies, including CMS, The Joint Commission, and the State of Louisiana.
- 2 years of experience in healthcare, patient safety, quality improvement, medical records, clinical care, revenue integrity, or data analysis.
- Licensed Registered Nurse through the Louisiana State Board of Nursing.
