**Project summary**:
Western populations are rapidly aging, with the consequent need for healthcare resources increasing. Alternatives to
conventional hospitalization are urgently needed to avoid complications related to hospitalization and to guarantee the
sustainability of the healthcare system. Previous studies have demonstrated the safety, efficacy and cost-effectiveness
of hospital at home (HAH) and comprehensive geriatric HAH (CG-HAH) models.

The heterogeneous and accelerated development of HAH units, especially since the COVID-19 pandemic, has hindered
their evaluation, urging the standardization of person-centered measures and outcomes. Patient and caregiver
perspectives can be assessed using reliable, valid and responsive instruments measuring outcomes (PROMs) and
experiences (PREMs), but existing evaluation tools must be assessed to determine which fit best with the needs of all
actors participating in HAH and CG-HAH.

Our main objective is to create a toolkit to evaluate HAH and CG-HAH from different perspectives: from health services
quality assessment and research, to individual patient monitoring. The toolkit will be the result of three main research
activities: 1) Quantitative evaluation of HAH utilization and outcomes in the healthcare region of Catalonia during the
last 5 years using whole-system data. 2) Qualitative assessment of the needs of all HAH actors: patients, caregivers,
